Infoline - United Way of Connecticut We examine all calls to this statewide program to detect trends in service needs and resources. This information is regularly disseminated to through the Infoline newsletter and special reports. Hartford School Readiness Council - Hartford Foundation for Public Giving Our report provides estimates of the future demand for school readiness programs and describes current capacity. Perinatal Information System - Hartford Health Department, March of Dimes, Hartford Women and Infants Health Network We are developing a city-wide information system covering births to Hartford mothers that will provide accurate, population-based data for planning, implementing and evaluating programs designed to improve the health of mothers and infants. This project was selected by CityMatCH to be a member of the 1999 Data Use Institute, co-sponsored by the US Centers for Disease Control and Prevention and the Maternal Child Health Bureau, Health Resources and Services Administration. Center for Children with Special Health Care Needs - Connecticut Children's Medical Center We provide periodic analyses and reports to describe the children served by the Center and the types of services they receive. Efforts are underway to link these data to other databases that capture information on special needs children in order to provide a more comprehensive picture of services that will lead to improvements in coordination and health care. Children's Home Care Network - Connecticut Children's Medical Center We are developing an information system for this program that will facilitate program management and improved service delivery. PROkids - Connecticut Children's Medical Center PROkids is an enhanced primary care program for infants who are drug exposed and their caregivers. The Child Health Data Center works with PROkids providers to develop and maintain information systems that support their mission of comprehensive care and research.
