For Dr. Milanese: Primary care providers, specialists and parents may refer children up to age 6 years who
have significant developmental problems. We honor most insurance and can give information regarding required
referrals when the initial appointment is scheduled.
Prior to the appointment, Dr. Milanese requests that parents complete a
background questionnaire to allow her
to get acquainted with the child’s history.
It is helpful to allow us to review any summary reports or letters from providers who have been involved with
your child’s care for the presenting concern. This information helps to provide a thorough evaluation and
comprehensive recommendations.
