The children's home care network provides a formal link
between the Medical Center and select home care providers to
coordinate and improve home care services offered to
patients and their families. Patients and providers may
choose home care services provided by skilled pediatric
staff that are family-centered, and accessible statewide
around the clock.
Under the direction of Discharge Planning, network members
are committed to collaboration in standards development,
staff development, and quality management to ensure the
delivery of high quality, cost effective, pediatric care in
